BROOKE & JUBAL'S OFFICE HOTTIE OFFICIAL CONTEST RULES
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED
1. "BROOKE & JUBAL'S OFFICE HOTTIE" contest (the "Contest") will begin at approximately 12:00 PM Pacific Time ("PT") on Thursday, September 1st 2011 and end at approximately 11:59 PM PT on Friday, September 23rd, 2011.
2. By submitting, voting or participating at any level you are agreeing to be bound by these contest rules.
3. How to Enter the Contest:
Registration for "Brooke & Jubal's Office Hottie" contest (the "Contest") will begin at approximately 12:00 PM Pacific Time ("PT") on Thursday, September 1st 2011 and end at approximately 11:59 PM PT on Friday, September 9th, 2011 changed to 5:00PM PT on Sunday, September 11th, 2011.
a. To secure your entry in the Contest you must enter via the following method.
i. Online: Beginning at approximately 12:00 PM PT on Thursday, September 1st 2011 go to www.movin925.com, click on the contest entry link, and complete an official entry form including your Candidate's First Name and Last Initial, Candidate's Date of Birth, Company name and office location and Email address, Submitter's Name, Submitter's Email address, Submitter's address including zip code and Submitter's Date of Birth. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an internet access provider, online service provider or other organization responsible for assigning email addresses or the domain associated with the submitted email address. For purposes of these official rules, receipt of an online registration will be solely determined when the www.movin925.com web server records the registration information. Any automated computer receipt (such as one confirming delivery of email) does not constitute actual receipt of a registration form for purposes of these official rules. MOViN 92.5's computer is the official time keeping device for this Contest.
4. All registration entries must be received by 11:59 PM PT on Friday, September 9th 2011 changed to 5:00PM PT on Sunday, September 11th, 2011.
a. Additional entry options may be made available at the sole discretion of MOViN 92.5/KQMV (the "Contest Entities").
b. Limit one (1) entry per candidate.
c. All submissions become the sole property of the Contest Entities and will not be returned.
d. The Contest Entities are not responsible for lost, stolen, damaged, mis-delivered, mangled, incomplete or late entries or for entries not received due to difficulty accessing the Internet, service outages or delays, computer difficulties or other technological glitches.
e. Each submission must not contain copyrighted materials or any other materials that may be restricted, prohibited or outlawed by federal, state or local laws.
f. The Contest Entities in their sole discretion, reserve the right to disqualify any submission if such submission contains illegal, indecent or otherwise offensive material, submissions that are not authorized by the candidate or if such act contains material whose content is deemed, at the sole discretion of the Contest Entities to be unsuitable for the public or broadcast on KQMV (i.e. pictures must be clean and no nudity.)
g. Any entries whose submission does not comply with these rules will be disqualified.
5. Eligibility Restrictions:
a. The contest is open to candidates who are a minimum of 18 years of age or older as of September 1st, 2011 residing in and whose office is located in the State of Washington.
b. Submitters must be a minimum of 18 years of age or older as of September 1st, 2011 residing in the State of Washington.
c. For the purposes of this contest, Winner's office will be defined as one geographic location located in the state of Washington.
d. Employees of the KQMV MOViN 92.5 (the "Station,") its licensee, its corporate parents, subsidiaries and affiliates, its advertising agencies, participating sponsors/promotional partners, other radio stations in the Seattle, WA metropolitan area, and the members of their immediate families and/or households are ineligible to participate or win. Subject to all applicable federal, state and local laws and regulations.
e. MOVIN 92.5 permits (1) "prize of value" winner per household per 30 days and (1) prize valued over $500 per household per 180 days. This includes all contest features, major contests, consolation prizes, special listener events, weekend giveaways, ticket windows, online contesting, random drawings and all other contesting on MOViN 92.5. This does NOT apply to movie premiere events (winning tickets to a MOViN 92.5 movie premiere will not count as a "prize of value" included in the 30-day waiting period) or MOViN 92.5 MVP "Points" program.
f. In the event a listener wins a "prize of value" and they (or someone in their household/family) wins another "prize of value" within the 30 days of winning the original prize, the 2nd prize will be forfeited, regardless of prize and/or value.
g. Contestants are required to provide truthful information and the Station will reject and delete any entry it discovers to be false or fraudulent. The Station will disqualify any entry from individuals who do not meet the eligibility requirements, and the Station will also delete any entry knowingly received from persons under the age of 13 in compliance with the Children's Online Privacy Protection Act.
6. Prizes: One Grand Prize will be awarded as described on the official Brooke & Jubal's Office Hottie Contest Prize webpage.
a. The prize will be awarded to the winning candidate and the candidate's co-workers.
b. All prizes are to be picked-up at MOViN 92.5 offices at 3650 131st Ave SE, Suite 550,. Bellevue, WA 98006. Winner will have 30-days to do so.
c. Any prize or prize certificate not claimed within thirty (30) days of winning will be forfeited by the winner. In the event that a prize or prize certificate is mailed to the winner, it will be with the prior written consent of the winner and therefore, winner assumes the risk of its loss. The Contest Entities are not responsible for the safe arrival of a prize or prize certificate.
d. There is no substitution, transfer, or cash equivalent for prizes, except that Contest Entities may, at their sole discretion, substitute prizes of comparable value or cash. Prizes are expressly limited to the item(s) listed as described above and do not include taxes, gratuities or any other expenses. Other restrictions may apply.
7. Selection of Winners:
a. Decisions of the Contest Entities with respect to the contest are final.
b. Each entrant meeting all eligibility and registration requirements of these official rules will be posted on www.movin925.com where listeners can view the entries.
c. All listeners of KQMV MOViN 92.5 wanting to participate in voting must sign up for the MOViN 92.5 MVP Club in order to participate in Brooke & Jubal's Office Hottie Contest Voting. If you are already a member of the MOViN 92.5 MVP Club you must update your record in order to vote. To sign up to be an MVP Club Member you must provide a valid email address.
d. When voting is open, listeners may only vote ten times per day per valid email address via their MOViN 92.5 MVP Club. Listeners may use their ten votes any way they see fit (i.e.: using all ten votes on one candidate or using all ten votes on multiple candidates.)
e. The Contest Entities are not responsible for votes not received due to difficulty accessing the internet, service outages or delays, computer difficulties or other technological glitches.
f. The Voting Schedule will be as follows:
i. Voting Period 1
1. Between Monday, September 12th, 2011 at 12:01 AM PT and Thursday, September 15th, 2011 at 11:59pm PT any listener registered to vote can go online to www.movin925.com and vote for their favorite candidate.
2. The fifty (50) candidates that receive the most votes during the first voting period will qualify for the next round of selection. In the event that there is a tie for the 50th position, then the station entities will select their favorite candidate to continue on.
ii. Voting Period 2
1. Between 12:01am PT and 11:59pm PT on Monday, September 19th , 2011 any listener registered to vote can go online to www.movin925.com and vote for their favorite candidate out of the top 50.
2. The twenty-five (25) candidates that receive the most votes during the voting period will qualify for the next round of selection. In the event that there is a tie for the 25th position, then the station entities will select their favorite candidate to continue on.
iii. Voting Period 3
1. Between 12:01am PT and 11:59pm PT on Wednesday, September 21st, 2011 any listener registered to vote can go online to www.movin925.com and vote for their favorite candidate out of the top 25.
2. The five (5) candidates that receive the most votes during the voting period will qualify for the next round of selection. In the event that there is a tie for the 5th position, then the station entities will select their favorite candidate to continue on.
iv. Voting Period 4
1. Between 12:01am PT and 11:59pm PT on Friday, September 23rd, 2011 any listener registered to vote can go online to www.movin925.com and vote for their favorite candidate out of the top 5.
2. The one (1) candidate that receives the most votes during the voting period will be selected as the Winner of Brooke & Jubals' Office Hottie Contest.
g. On Monday, September 26th, 2011 at TIME TBA- the winner of Office Hottie will be announced via on-air during the Brooke & Jubal's Morning Show.
h. Winners need not listen nor be present to win.
i. The winners will also be contacted by the Contest Entities via telephone the week of September 26th, 2011.
j. Odds of winning will be determined by the number of eligible votes and entries received.
k. Winners must execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within thirty (30) days of notification attempt or prize will be forfeited and an alternate winner will not be chosen.
l. Winners will receive a winners release form from MOViN 92.5 and are subject to the conditions outlined on the acknowledgement forms. If a winner is deemed ineligible, he or she will be contacted by telephone. The information provided by the winner will be kept in confidence by MOViN 92.5 and will not be disclosed to any third party excluding the IRS if prize accumulation exceeds $600 in a calendar year or in the event of a Bellevue Radio Inc. audit.
m. By accepting the prize, the winner releases MOViN 92.5 and all sponsors/participants from any/all physical liabilities resulting in injury or death.
n. Winner certifies he/she has read, understands and will abide by all contest rules.
o. Winners must meet all eligibility requirements.
p. Winner will look solely to the manufacture and /or the end supplier for goods and services for all warranty and liability claims.
8. Conditions:
a. Payments of all federal, state and local taxes are solely the responsibility of the winner(s). Winners will be required to complete and submit an IRS Form W-9 with the winner's full Social Security Number or the equivalent for receipt of any prize valued at $600 or more or for any prizes awarded by the Station in a calendar year with an aggregate value of $600 or more. Failure to submit a complete W-9 or equivalent will result in forfeiture of the prize.
b. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize.
c. Prior to awarding any prize or prize certificate the Station in its sole discretion may require contest winners to sign a liability release, agreeing to hold the Station, its corporate licensee, its parent, subsidiary and affiliated corporations and the officers, shareholders, directors, employees, agents and representatives, its advertising agencies, participating sponsors and promotional partners to all be held harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest.
d. The Station, in its sole discretion, reserves the right to disqualify any person tampering with the entry process; the operation of the Station's Web Site or is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the Contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort.
e. The Contest Entities reserve the right to make changes in the rules of the contest, including the substitution of a prize or prizes of equivalent value, which will become effective upon announcement. If due to circumstances beyond the control of the Station, any competition or prize-related event is delayed, rescheduled, postponed or cancelled, the Station reserves the right, but not the obligation, to cancel or modify the Contest and shall not be required to award a substitute prize.
f. Failure to comply with the contest rules may result in a contestant's disqualification, at the sole discretion of Contest Entities.
g. The Contest Entities are not responsible for typographical or other errors in the printing, the offering or the administration of the contest or in the announcement of a prize.

9. Internet Contest Privacy Policy
a. By use of this website and by entering the Contest, you agree to the MOViN 92.5 Web Site Terms of Use Agreement and to the use of your personal information as described in the Privacy Policy located at: www.movin925.com
